Sprains and strains are some of the most common injuries, from a rolled ankle on the court to a pulled muscle while lifting something heavy. Most heal with simple care at home, but the right first steps speed recovery, and a quick check can rule out a more serious injury such as a fracture.

Sprain or Strain? Knowing the Difference

The two injuries are related but not the same. A sprain is a stretched or torn ligament, the tough tissue that connects one bone to another. The ankle is the most common spot for a sprain, though the wrist and knee are also frequent. A strain is an injury to a muscle or to a tendon, the tissue that connects muscle to bone, and strains often happen in the back or hamstring. Both injuries cause pain and swelling and are treated in similar ways in the first few days.

Common Symptoms

Signs of a sprain or strain include:

  • Pain and tenderness around the joint or muscle
  • Swelling, and sometimes bruising
  • Reduced range of motion, stiffness, or weakness
  • With a sprain, sometimes a pop felt at the moment of injury and trouble putting weight on the joint
  • With a strain, muscle spasms or cramping

The severity ranges from a mild stretch that is sore for a few days to a complete tear that needs more involved treatment.

Common Causes

These injuries usually happen when a joint is forced beyond its normal range or a muscle is overstretched. Common causes include:

  • Sports and exercise, especially with sudden stops, pivots, or jumps
  • Slips, trips, and falls
  • Lifting heavy objects or using poor technique
  • Overuse and repetitive motion
  • Not warming up before activity

What You Can Do at Home: R.I.C.E.

For most mild sprains and strains, start with the R.I.C.E. approach in the first day or two:

  • Rest the injured area and avoid activities that cause pain
  • Ice the area for short periods, with a barrier between the ice and your skin, to reduce swelling
  • Compression with an elastic bandage for gentle support
  • Elevation, keeping the injured area raised above heart level when you can

Over-the-counter pain relievers can help. Follow the label, and check with a provider if you are unsure. As the pain eases, gentle movement and a gradual return to activity usually help recovery. Avoid pushing through sharp pain.

Red Flags: When to Get It Checked

See a provider if you cannot put weight on the joint or use the limb, the area looks crooked or deformed, swelling or pain is severe, you feel numbness or tingling, the joint feels unstable, or the injury is not improving after a few days of home care. An exam and, when needed, an X-ray can rule out a broken bone and guide treatment such as bracing, crutches, or a referral. Get emergency care for an obvious deformity, a bone that has broken through the skin, or a limb that is cold, pale, or numb.
